This is the deposit form for the 2023/2024 CYO Boys Rep Team Basketball program.

Please complete the form below by filling out all of the required fields. After completing the deposit form, click on the SUBMIT button for additional instructions.

All rep players must pay a $250 deposit towards team expenses. The deposit cost for one REP TEAM participant for the 2023/2024 season is $250. Your balance payment will be due at the end of the season and after fundraising and sponsor revenue is calculated.
